I’m not sure if you are familiar with the sick note which issued by real doctor if this is the first time for you to looking for it. That why I provide this sample of doctors sick note here. But for those who has been obtained it before, that going to be easier for them to remember its format without any problem.

Well, before I write more about this example or before you download and view it, I would like to emphasize that there are some limitation of this so called doctor’s sick note example. So, all of you will be aware what happened if you cross the boundary after this. First of all, this note created in purpose of giving you an overview of doctors excuse note and what it looks like in the first place. If you haven’t notice, it should have a unique logo of the issuance clinic or hospital as well as their address and contacts such as phone number, website or email if applicable. This address or contact shall be relevant, which means it near to your house, school or working area. If you live in United Kingdom but your sick note has an address and contact number from other country, it might very look suspicious right?

In addition, real medical certificate should have a date of the day patient meet the doctor, diagnostic of the suffered disease, numbers of day leave given and of course the patient full name. In most cases, sick note will have a doctor’s or medical officer signature at the bottom of the sick leave certificate to make it valid for official use such as getting excused from perform duty at work, attending school classes, exam or anything that necessary this kind of procedure. That are things suppose to be appear in a good or real doctor’s excuse.

Now, we get back to this sample thing here. As I mentioned earlier, it certainly cannot be used for official matters or to get real excuse from real situation. If you still want to pursue it, I can’t guarantee that you aren’t get busted. Seriously, it suitable for references, entertainment purpose only. However, it can be fun too if you want to thrill your parent, grandma or siblings by showing them that this fake sick note is genuine one, it might works. But I am not going to responsible for the bad consequences like getting scolded or even worst get punished or canned on your ass. Do it on your own risk
