I’m not sure if you are familiar with the sick note which issued by real doctor if this is the first time for you to looking for it. That why I provide this sample of doctors sick note here. In most cases, sick note will have a doctor’s or medical officer signature at the bottom of the sick leave certificate to make it valid for official use such as getting excused from perform duty at work, attending school classes, exam or anything that necessary this kind of procedure. That are things suppose to be appear in a good or real doctor’s excuse.

However, it can be fun too if you want to thrill your parent, grandma or siblings by showing them that this fake sick note is genuine one, it might works.
